| CORPORATION COVERAGE | - This policy will ONLY extend coverage to the named Independent Contractor under contract with the staffing agency designated in your application. It WILL NOT cover any other independent contractors, employees, or partners of your corporation. Any other contractors who require coverage must purchase their own policy.
| - This policy extends coverage to your whole corporation, including all directors, staff, and hired contractors (contractors must be individuals and not incorporated).

| POLICY PERIOD | - Flexible policy periods, ranging anywhere from from 1 week to 18 months.

| CANCELLATION TERMS | - Flexible cancellation with:
- A full refund if the policy is cancelled before the effective date; or
- A prorated refund if cancelled after the effective date, provided there is no claim on the policy.
| - No refund for cancellation. If the policy is cancelled, your premium will be fully retained by PROLINK.

| COST | - Rates are determined based on your role (e.g. IT, Management Consulting, etc.), plus an additional policy fee; revenues are not a consideration factor for your premium.
| - Rates are determined based on your total annual revenue.

| PROGRAM ELIGIBILITY | - In order to qualify for this program:
- You do not need to be a member of a professional association.
- You must contract with only ONE specified staffing company that is participating in PROLINK’s program.
- You will not qualify for the program if:
- You contract with multiple staffing companies.
- You work independently/directly with clients.
- You are no longer contracting with a staffing company that is part of PROLINK’s program.
- You are in a role that has signing authority, final decision-making authority, or supervisory responsibilities.
- You are subject to one of the Administrative/Clerical or Accounting/Finance or Technical/Engineering Exclusions.
- If you are unable to participate in the ICP program, you may be eligible for coverage through the APCC Professional Insurance Program or another one of PROLINK’s Professional Insurance Programs.
| - You must be a member of the Association of Professional Canadian Consultants (APCC) to be eligible for this coverage.
